1. Grant of Rights
By signing this form, I hereby grant Stratus Technology Services LLC (“Stratus”) and its affiliates the irrevocable, worldwide, royalty-free right to:
Record, use, edit, and distribute my testimonial, likeness, voice, and statements provided during the video shoot.
Display, reproduce, and publish the testimonial in any format, including but not limited to digital, print, social media, and broadcast channels, for marketing, promotional, and educational purposes.
2. Company Representation
I confirm that I am authorized to represent my organization and provide this testimonial on its behalf.
3. Logo and Brand Asset Usage
As of the Effective Date, Partner hereby grants Stratus a non-exclusive, royalty-free, revocable license to use its logo, trademark, and brand name (“Brand Assets”) for marketing and promotional purposes, including but not limited to:
Websites, social media, sales collateral, trade show materials, and co-branded campaigns.
All usage will adhere to Partner’s brand guidelines and will be subject to review upon request.
This authorization is valid for the duration of the partnership or until revoked in writing with 30 days’ advance notice from Partner.
Stratus agrees to use the Brand Assets in a manner that maintains the integrity and reputation of Partner.
4. No Compensation
I acknowledge that no monetary compensation will be provided for the use of my testimonial, company name, or logo.
5. Release of Liability
I release Stratus, its employees, agents, and affiliates from any claims, demands, or liabilities arising out of or in connection with the use of the testimonial, company name, or logo as described above.
